Abortion Drugs Ease - FDA

FDA Eases Restrictions on Abortion Medication



 The U.S. Food and Drug Administration is making it easier for women to obtain a medication that induces abortion.
Under changes announced Wednesday by the agency, women in most states who want the drug, called mifepristone (Mifeprex), will need to make just two trips to a doctor, instead of three.
They'll also have 10 weeks from the start of their last period to use the medication to terminate pregnancy, up from the prior cutoff of 49 days, The New York Times reported.
Additionally, the FDA is reducing the drug's dosage from 600 milligrams to 200 mg, reportedly reducing the cost of mifepristone and its potential side effects.
Abortion-rights advocates welcomed the label changes.
"Unlike state and federal anti-choice laws that restrict a woman's access to an abortion, the FDA decision to make mifepristone available to more women in this country is based on science that shows this usage is safe and medically sound," said Ilyse Hogue, president of NARAL Pro-Choice America, in a statement. "It is high time to put medicine back into the hands of medical professionals and take it out of politicians' hands."
An anti-abortion group took a different view, however.

"The new label affirms the deadly realities of chemical abortion and underscores the need for in-person patient examination and follow-up care as well as the fact that the abortion drug regimen presents serious risks to women's health," Anna Paprocki, staff attorney for Americans United for Life, told the Washington Post.
But abortion rights advocates and many physicians said the original abortion-medication labels were outdated and contradicted sound medical practice. They also had argued that states such as Texas, North Dakota and Ohio -- which required physicians to follow the label restrictions -- erected barriers to abortion, the Times said.

"This is a huge step in increasing access to medication abortion and it comports with the scientific evidence," Elizabeth Nash, a senior state issues associate at the Guttmacher Institute, which tracks women's reproductive health issues, told The Times. According to the institute's latest figures, medication abortions accounted for about 25 percent of all abortions in 2011, she said.
The American Congress of Obstetricians and Gynecologists also backed the FDA decision. "The updated FDA-approved regimen for mifepristone reflects the current available scientific evidence and best practices," it said in a statement.
The drug, formerly known as RU-486 and approved by the FDA in 2000, works by blocking receptors of progesterone, a hormone needed in pregnancy, and is used with the drug misoprostol to end a pregnancy.

More Evidence Diabetes Drug Actos Raises Bladder Cancer Risk a Bit

 More evidence linking the diabetes drug Actos to an increased risk of bladder cancer has surfaced in a new study that also finds the risk rises with increased use.

Actos (pioglitazone) appears to increase risk of bladder cancer by 63 percent, Canadian researchers say. The findings, published March 30 in The BMJ, stem from an analysis of nearly 146,000 patients treated between 2000 and 2013.

The data also showed that bladder cancer risk increases if Actos is used for more than two years, or if someone takes more than 28,000 milligrams over the course of their lives.

The overall risk is small, however. Still, "the more you accumulate the drug into your system, the higher the risk," said senior researcher Laurent Azoulay, an associate professor of oncology at McGill University in Montreal.

Azoulay and his colleagues did not find a similar link between bladder cancer and Avandia (rosiglitazone), another diabetes drug in the same class of medication as Actos.
"It appears to be a drug-specific effect, not a class effect," Azoulay said.

The maker of Actos, Takeda Pharmaceuticals, issued a strong statement in response to the study.
"Takeda is confident in the positive benefit-to-risk profile of pioglitazone. Two large, long-term observational studies found no significant increase in the risk of bladder cancer in diabetic patients taking pioglitazone. This data includes a 10-year, prospective cohort study, conducted by the University of Pennsylvania and Kaiser Permanente Northern California and ... a large epidemiological study utilizing five European Union databases ... to investigate the potential risk of bladder cancer with pioglitazone use," said Elissa Johnsen, head of the company's Global Product and Pipeline Communications.

Actos and Avandia are thiazolidinediones, a class of drug that helps lower blood sugar by helping the body's cells more effectively use insulin.
Both drugs have been around since the late 1990s in the United States, Azoulay said, and each has had a troubled history.

Previous studies have linked Avandia to heart failure and heart disease, while in 2005 a clinical trial unexpectedly showed an increase in bladder cancer cases among patients taking Actos, he said.
Since then, the association between Actos use and bladder cancer has been controversial, with studies reporting contradictory findings, the study authors said in background notes.

For the study, they explored the link between Actos and bladder cancer by analyzing data from the UK Clinical Practice Research Database for 145,806 patients newly treated with diabetes drugs between 2000 and 2013. They took into account other risk factors such as age, sex, duration of diabetes, smoking status and alcohol-related disorders.

When compared with other classes of diabetes drugs, the higher risk of bladder cancer linked to Actos was significant, the study found.

The reason Actos, but not Avandia, is associated with bladder cancer may come down to the fact that there are key differences between the two drugs, Azoulay said.

Actos targets two different receptors to make cells more sensitive to insulin, while Avandia targets only one. The additional receptor influenced by Actos could be the reason for increased bladder cancer risk, he said.
The controversy over Actos and Avandia is largely moot at this point, diabetes experts say, because newer and safer medications have supplanted them.

"When these drugs were first approved in the U.S., they were one of the very few options we had," said Dr. Kevin Pantalone, an endocrinologist with the Cleveland Clinic. "Now, largely because of the controversy surrounding these agents in recent years, their prescription pattern has declined."
In 2008, thiazolidinediones accounted for 20 percent of diabetes drug prescriptions handed out by the Cleveland Clinic, Pantalone said. Five years later, in 2013, they accounted for only 7 percent of diabetes prescriptions.

Dr. Caroline Messer, director of the center for pituitary and neuro-endocrine disorders at Lenox Hill Hospital in New York City, agreed there's little call for either Actos or Avandia these days.
"I don't think I've used Actos since 2005, to be honest, or I've used it rarely," Messer said. "I have so many other medications at this point, there's not a lot of reason for me to reach for this."

Actos already carries an FDA-mandated warning on its label about bladder cancer risk, Azoulay said.
But even with this new evidence, diabetes experts said they still want Actos available as an option for treatment.

Bladder cancer is a rare disease, and remains rare even after Actos increases its risk, Pantalone said.
Decisions about its use should be made according to the individual patient, said Dr. Robert Courgi, an endocrinologist with Northwell Health's Southside Hospital in Bay Shore, N.Y.

"Actos is a potent, inexpensive oral drug for diabetes that should not be pulled off the shelves," Courgi said. "The clinician must decide if Actos is the right drug for the patient. Obviously, patients with a history of bladder cancer or who are at high risk should not get Actos."

Smoking can dramatically change the balance of bacterial species in the mouth, which may affect the risk of mouth, lung and digestive system diseases, a new study says.
The research also found that the proper mix of bacteria in the mouth is restored if people quit smoking.

Researchers analyzed the mix of about 600 bacterial species in the mouths (oral microbiome) of more than 1,200 Americans. All were 50 or older. The group included smokers, former smokers and people who never smoked.

Current smokers had significantly increased growth of more than 150 bacterial species, while 70 other species showed sharp decreases in growth. For example, Proteobacteria species accounted for less than 5 percent of overall bacteria in the mouths of smokers, compared with about 12 percent in nonsmokers, the researchers reported.

Proteobacteria help break down toxic chemicals introduced by smoking, the researchers said.
Compared with nonsmokers, smokers had 10 percent more species of Streptococcus, which promotes tooth decay, the study authors said.

The good news is that the researchers found that mouth bacteria seem to return to normal after a person quits smoking, although it's not clear how long that takes. All former smokers who had not smoked for at least 10 years had the same balance of bacterial species as nonsmokers, the study authors said.
"Our study is the first to suggest that smoking has a profound impact on the oral microbiome," said study senior investigator and epidemiologist Jiyoung Ahn, an associate professor at NYU Langone Medical Center, in New York City.

"Further experiments will be needed, however, to prove that these changes weaken the body's defenses against cancer-causing chemicals in tobacco smoke, or trigger other diseases in the mouth, lungs or gut," Ahn said in a center news release.

Previous research has linked bacterial species imbalances in the digestive system to immune disorders such as Crohn's disease, as well as some gastrointestinal cancers, according to background notes with the study. More than three-quarters of oral cancers are associated with smoking, but it's not known if smoking-related changes to bacterial populations in the mouth contribute to the risk of these cancers.
The study was published recently in the ISME Journal.

Study Shows That Endometriosis May Link to Heart Disease

Endometriosis Linked to Heart Disease in Study



 Women who have endometriosis, the abnormal growth of uterine tissue outside the uterus, may face a 60 percent higher risk of developing heart disease than women without the disorder, a new study suggests.

The potential risk was especially high for women who were 40 or younger: they were three times more likely to have heart disease than women in the same age range without the gynecological condition, the researchers found.

That finding could be partly explained by the endometriosis treatments themselves. These treatments, such as removal of the uterus and ovaries, have been linked in other studies to potential heart disease risk, the study authors said.

"Women with endometriosis should be thinking about lifestyle changes and discussion with their doctor about steps they can take to prevent heart disease," said Stacey Missmer, the study's senior author. Missmer is director of epidemiologic research in reproductive medicine at Brigham and Women's Hospital in Boston.
Missmer said the study found an association between endometriosis and the risk of heart disease, but can't prove a cause-and-effect relationship. She believes this is the first study to follow people over time, noting, "I would argue that no first study is clear evidence of cause and effect."
However, other studies support the link between endometriosis and heart issues, she said.

"We knew that women with endometriosis have a chronic inflammation. There was one study previously that suggests women with endometriosis have a poor lipid profile -- high 'bad' cholesterol and low 'good' cholesterol," Missmer said. These patients also have high levels of oxidative stress. All of those conditions are tied to heart disease risk, she said.

Endometriosis affects about 10 percent of women of reproductive age in the United States, according to background information in the study. Symptoms include pelvic pain and painful periods. The condition is known to reduce fertility.

The study, published online March 29 in Circulation: Cardiovascular Quality and Outcomes, looked at more than 116,000 women enrolled in the Nurses' Health Study II. The researchers found nearly 12,000 women who received a diagnosis of endometriosis during the 20-year follow-up, which ended in 2009.
In addition to looking at heart disease, Missmer's team found that women with endometriosis were 1.35 times more likely to need surgery or stents to unblock arteries. These patients were also 1.5 times more likely to have a heart attack, and nearly two times as likely to have chest pain, or angina.

Younger women had an even higher risk of heart disease, the researchers found. Those 40 and younger with endometriosis were three times as likely to have a heart attack, need surgery for a blocked artery or have chest pain (angina) compared to those of the same ages without endometriosis.

Other studies have found that removal of the uterus and ovaries may increase the risk of heart disease, according to the researchers. As women in the study aged, Missmer said, the influence of the endometriosis on their heart disease risk went down, probably because other risk factors -- such as high cholesterol or high blood pressure -- came into play, explaining much of the risk.

The study finding "is not a surprise to me," said Dr. Tamer Seckin, a New York City-based endometriosis
surgeon and gynecologist who co-founded the Endometriosis Foundation of America.

"This is significant, that they have statistically found an important link between endometriosis and coronary heart disease," Seckin said. The study is strong for several reasons, including its length of follow-up, he added.

Based on the new study findings, Seckin said, he plans to pay more attention to his patients' risk of coronary heart disease, asking about family history and other risk factors.

Seckin advocates treating endometriosis, whenever possible, with conservative surgery that removes the lesions, but not the uterus or ovaries.

Missmer stressed that "the take-home message here is that heart disease is the leading killer of women in the U.S. One in three women will experience heart disease, and women need to understand that whether you have endometriosis or not, you should be paying attention to healthy lifestyle choices, even if you are young."

B. Braun Medical Inc. is recalling one lot of 5% Dextrose Injection USP 100/150mL container (Lot #J5J706, catalog #S5104-5264, NDC 0264-1510-32) to the consumer level. B. Braun recently identified an adverse quality trend in customer complaints reporting that some containers in lot J5J706 exhibited leakage and, in a few instances, visible particulate matter identified to be microbial growth.

A compromise of container integrity has the potential for leakage of the solution, usually identified prior to the use of the product. Leaking containers allow contamination of the solution, which can and has led to microbial contamination. Intravenous administration of a non-sterile product can result in serious infections that may be life-threatening.

BACKGROUND:
This product is intended for intravenous administration and may be used as a diluent and delivery system for intermittent administration of compatible drug additives. The product is packaged in B. Braun's PAB (Partial Additive Bag) container with 64 units per case. The affected lot J5J706, which expires 10/31/2016, was distributed nationwide to licensed distributors, hospitals, and pharmacies.

RECOMMENDATION:
 B. Braun is notifying its distributors and customers by certified mail and is arranging for the return of all recalled product. Distributors and customers that have inventory of lot J5J706, 5% Dextrose Injection USP 100/150mL container, should discontinue use immediately and contact B. Braun Medical Inc.'s Customer Support Department at 1-800-227-2862 Monday through Friday, 8 a.m. to 6 p.m. (EST) for instructions for returning the affected product and to arrange for replacement product.

Infant Ear Infections Becoming Less Common




Painful ear infections remain a scourge of childhood, but fewer American babies are getting them now compared with 20 years ago, new research shows.

The study didn't dig into the reasons for the decline. But experts say the credit likely goes to certain childhood vaccines, rising rates of breast-feeding and the drop in Americans' smoking rate.
The new research found that 46 percent of babies followed during 2008 to 2014 had a middle ear infection by the time they were 1 year old.

But while the infections were common, those rates were lower when compared against U.S. studies from the 1980s and '90s, the researchers added. Back then, around 60 percent of babies had suffered an ear infection by their first birthday, the study authors said.

The decline is not surprising, according to lead researcher Dr. Tasnee Chonmaitree, a professor of pediatrics at the University of Texas Medical Branch, in Galveston.
"This is what we anticipated," she said.

That's in large part because of a vaccine that's been available in recent years: the pneumococcal conjugate vaccine, Chonmaitree said. The pneumococcal conjugate vaccine protects against several strains of pneumococcal bacteria, which can cause serious diseases like pneumonia, meningitis and bloodstream infections.
Those bacteria are also one of the major causes of children's middle ear infections, Chonmaitree said.
She added that yearly flu shots, which are now recommended for children starting at the age of 6 months, are probably helping, too: Ear infections often arise after a viral infection like the flu or common cold, the study authors said.

Vaccinations "could very well be one of the drivers" behind the decline in infant ear infections, agreed Dr. Joseph Bernstein, a pediatric otolaryngologist who wasn't involved in the study.
But there are other important factors, too, both he and Chonmaitree said -- namely, rising rates of breast-feeding and a decrease in babies' exposure to secondhand smoke.

"The data really do suggest that breast-feeding -- particularly exclusive breast-feeding in the first six months of life -- helps lower the risk of ear infections," said Bernstein, who is director of pediatric otolaryngology at the New York Eye and Ear Infirmary of Mount Sinai, in New York City.

Why does breast-feeding matter? The main reason, Chonmaitree said, is that breast milk contains antibodies that can help protect babies against infections.
There's also the fact that breast-fed babies are less likely to spend time drinking from a bottle while lying down, Bernstein noted. That position can make some infants more vulnerable to ear infections, he said.

The study findings were based on 367 babies followed during their first year of life. By the age of 3 months, 6 percent had been diagnosed with a middle ear infection; by the age of 12 months, that had
risen 46 percent, researchers found.

Not surprisingly, babies with ear infections also suffered more colds during their first year -- almost five, on average, versus two among babies who didn't develop an ear infection, the study authors said.
Breast-fed babies had a lower ear infection risk, however. Those who'd been exclusively breast-fed for at least three months were 60 percent less likely to develop an ear infection in their first six months, the study showed.

To Chonmaitree, the findings underscore evidence that breast milk is the best nutrition for babies. "Breast-feeding is good," she said. "Parents should be encouraged to do it if they can."
Bernstein stressed the "if they can" part. Some mothers, he noted, simply can't exclusively breast-feed for a long time -- because of work or other reasons.
But whether babies are breast-fed or not, they will benefit from routine vaccinations, Chonmaitree said. "Parents should make sure they're on schedule with the recommended vaccines," she said.

According to the American Academy of Pediatrics (AAP), most children with middle ear infections get better without antibiotics, and doctors often recommend pain relievers -- like acetaminophen -- to start. But with babies, Bernstein said, antibiotics are often used right away.
The AAP recommends antibiotics for infants who are 6 months old or younger, and for older babies and toddlers who have moderate to severe ear pain.

The study was published online on March 28 in the journal Pediatrics.

quality of care would be the same as in a hospital, the researchers pointed out.

"The cancer patient and family tend to be concerned that the quality of medical treatment provided at home will be inferior to that given in a hospital and that survival might be shortened," said study author Dr. Jun Hamano, of the University of Tsukuba in Japan.

"However, our finding -- that home death does not actually have a negative influence on the survival of cancer patients at all, and rather may have a positive influence -- could suggest that the patient and family can choose the place of death in terms of their preference and values," Hamano added.
For the study, the researchers looked at almost 1,600 patients who died in hospitals and almost 500 who died at home.

The investigators found that those who spent their last days at home lived considerably longer.
The study results appear in the March 28 online issue of the journal Cancer, which is published by the American Cancer Society.

"Patients, families, and clinicians should be reassured that good home hospice care does not shorten
patient life, and even may achieve longer survival," Hamano said in a journal news release.

Study: Blood Test May Help Diagnose Concussions




A simple blood test appeared to detect signs of concussion up a week after patients suffered a head injury, researchers report.
The test might provide a new way to diagnose concussions, especially in patients without immediate symptoms, the researchers said.

However, the research is preliminary and the test would not be ready for widespread use for several years, experts said.
"Symptoms of a concussion, or a mild to moderate traumatic brain injury, can be subtle and are often delayed, in many cases by several days," said study lead author Dr. Linda Papa, an emergency medicine physician and researcher at Orlando Health.

"This [blood test] could provide doctors with an important tool for simply and accurately diagnosing those patients, particularly children, and making sure they are treated properly," she added in a news release from the Florida health care provider.

The test looks for a biomarker called glial fibrillary acidic protein (GFAP), which is released into the bloodstream when the brain is injured. In nearly 600 patients age 18 and older, the blood test detected mild to moderate concussion with up to 97 percent accuracy, the researchers said.

"Not only were they present in the bloodstream, we were able to detect measurable levels of GFAP up to a week after the injury," said Papa, a U.S. National Institutes of Health-funded researcher.
The test could also help to identify patients who need neurosurgery and could significantly reduce the use of CT scans to diagnose concussions, according to the study published online March 28 in the journal JAMA Neurology. Though precise, CT scans are costly and expose patients to radiation.

An estimated 250,000 U.S. children are treated in hospitals each year for concussions from playing sports -- nearly 700 a day. Almost all concussions sustained by kids are diagnosed only by symptoms, such as vomiting, loss of balance, blurred vision or headaches. As a result, it can be difficult for doctors to determine the severity of the injury, Papa said.

"If patients are not diagnosed properly and treated appropriately, it could lead to long-term problems," she said, such as headaches, dizziness, memory loss and depression.
"We have so many diagnostic blood tests for different parts of the body, like the heart, liver and kidneys, but there's never been a reliable blood test to identify trauma in the brain. We think this test could change that," Papa said.
